Autistic children have high rates of emotional and behavioral disorders. Often, these disorders emerge when autistic children are young, < 6 years, and negatively impact developmental functioning and long-term outcomes (Green et al., 2012). A variety of disorder-specific psychotherapeutic interventions have garnered empirical-support (Dickson et al., 2021). While promising, existing trials have several limitations. First, autistic children demonstrate lower rates of remission in response to some disorder-specific treatments compared to non-autistic children receiving similar interventions (Warwick et al., 2017). Additionally, disorder-specific approaches require autistic children with co-occurring conditions to receive multiple interventions. Finally, most treatment models have been developed for older children, with limited options for young autistic children (Vasa et al., 2020). Hence, novel treatment approaches are sorely needed to advance mental health care for autistic children.
One potential strategy to improve mental health outcomes is to develop interventions that target transdiagnostic mechanisms (Kendall et al., 2020). Several mechanistic factors are implicated in the development and maintenance of anxiety, mood, and behavioral disorders in autistic children. These include parental/family accommodation, intolerance of uncertainty, emotion regulation, and cognitive flexibility (Conner et al., 2023; Feldman et al., 2019; Jenkinson et al., 2020). Targeting these mechanisms, instead of specific disorders, may produce larger effect sizes and offer a more efficient delivery model. Outcomes may also be improved by treating mental health conditions when autistic children are young, as this capitalizes on children’s heightened brain neuroplasticity and the increased malleability of parent-child response patterns in early development.
This symposium will present several mechanistic targets and treatment models for young autistic children. The first presentation will examine family accommodation as an underlying mechanism in the expression of repetitive behaviors and the use of ecological momentary assessment to differentiate adaptive/maladaptive repetitive behaviors (Shir Rozenblat, MA). The second presentation will discuss the feasibility and preliminary efficacy of DINO Strategies for Anxiety and fear of Uncertainty Reduction (DINOSAUR), a virtual anxiety treatment for children and parents targeting parental accommodation and intolerance of uncertainty (Amy Keefer, PhD). The third presentation will present changes in emotional regulation in response to the Stress and Anger Management Program (STAMP), a clinic-based, group intervention for children and parents (Jennifer Bertollo, MS). The fourth presentation examines mental health and behavioral outcomes following a preschool adaptation of the Unstuck and On Target intervention (UoT), a child-focused treatment targeting emotional regulation and cognitive flexibility in school and clinic-based settings (Allison Ratto, PhD). Finally, the Discussant (Conner Kerns, PhD) will discuss findings in the context of the current landscape of mental health research and present future directions for intervention development.
